Log a nonconformance (NCR), work from the symptom to the real cause with a guided 5 Whys and a fishbone that draws itself as you type, assign corrective and preventive actions (CAPA), and track it to closure, all saved against your account.
Describe the non-conformance factually: what was observed, on which product, and where it was caught. A precise problem statement is what keeps the analysis honest.
Each answer becomes the question for the next line. Stop when the answer points to a process or system cause you can actually change, not a person to blame. You do not have to fill all five.
5 Whys follows one thread. The fishbone makes sure you considered every category of cause. Add contributing factors under each of the 6M headings; the diagram updates live.
Pull the threads together into the one or two underlying causes the evidence actually supports. This is what the corrective action must eliminate.
Containment stops the bleeding now. Corrective action removes the root cause. Preventive action stops it recurring on other jobs or lines.
An RCA is not closed until you have proof the fix held. Define how you will check and when.
