Define the problem

Describe the non-conformance factually: what was observed, on which product, and where it was caught. A precise problem statement is what keeps the analysis honest.

Ask why, five times

Each answer becomes the question for the next line. Stop when the answer points to a process or system cause you can actually change, not a person to blame. You do not have to fill all five.

State the root cause

Pull the threads together into the one or two underlying causes the evidence actually supports. This is what the corrective action must eliminate.

Corrective & preventive action

Containment stops the bleeding now. Corrective action removes the root cause. Preventive action stops it recurring on other jobs or lines.

Verify it worked

An RCA is not closed until you have proof the fix held. Define how you will check and when.
